The basis of successful diabetes self-management is an understanding of the diabetes process and a sound knowledge of the treatment plan.
Diabetes Educators work in partnership with General Practitioners, other members of the multidisciplinary team and the person with diabetes to achieve mutually agreed diabetes management goals.
The self-management model of care promotes access to and coordination of community health services to provide:
- Diabetes information/day to day management and support; diabetes equipment
- Education – how diabetes affects your body & achieving diabetes control
- Assistance in monitoring and maintaining healthy blood glucose levels
- Support self-management
- Referral information e.g. referral to other services such as community programs
- Liaison with local and regional General Practitioners, Physicians, Endocrinologists
- Assistance in coping with the impacts of long term health condition
