Complex Care (formally hospital at risk program – HARP)

Description

Portland District Health’s Complex Care service helps people who live in the Glenelg Shire and who have long term illness such as heart or breathing complaints, diabetes or memory issues that affect how you or your carer cope at home.

Staff support you to identify the health and well-being areas that could be improved and support you to live in better health.  We do this by teaching you how to understand warning signs that you are getting ill and by linking you into any community services that you may need.

The service is short term and free.  Staff can visit you in your own home, if that is where you feel most at ease.  Staff help you to know when your health is going downhill, develop an action plan with you and your support team so that unnecessary hospital admissions can be reduced.
