The Transition Care Program (TCP) provides older people with up to 12 weeks of additional time in hospital or at home to recover from illness and to maintain or improve their level of function. TCP provides time and support to consider long term care choices, extended time working with occupational therapy & physiotherapy, assistance to continue living safely at home, nursing & home care services and health care coordination. ACAS is required.
TCP is located in the Sub-Acute ward, 1st floor of the Portland Hospital. Four beds are located within the ward environment and two are in the community.
