Community Aged Care Services (CACS)
Portland District Health provides home support for people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people) to remain living at home and in their communities.
The program places a strong focus on activities that support independence and social connectedness and consider each person’s individual goals and choices. We work with the family to understand the needs of each person.

Fees will be discussed on admission.
