Early Intervention Chronic Disease

Description

Being told that you have a chronic disease can be confusing and overwhelming. You may not look or even feel sick. However if the condition is not managed in the early stages, your health could deteriorate and that is when you could start to feel sick. Knowing where to get support or where to find out more information about your condition can be confusing.

At Portland District Health we have a range of allied health professionals who are able to provide you with individualised support that will assist you to become more confident in self-managing your condition.

 

Early Intervention in Chronic Disease Services may be accessed directly by people with a chronic disease, their family members and care givers.

Referrals from your GP or other health professional may be emailed or faxed
