The Chronic Disease Management Program currently includes Pulmonary Rehabilitation and Cardiac Rehabilitation – both are managed under the Chronic and Complex Care team.
The aim is to assist people with chronic diseases to become independent at home with a focus on self-management. These program is open to anyone with a chronic disease. Rehabilitation is provide either within a group based setting or one on one appointment, this will be discussed on admission to program by the CDMP coordinator.
