Glenelg Shire Council recently stepped away from delivering aged and disability services, with the Commonwealth Government appointing Portland District Health, Heywood Rural Health, and Western District Health Service as new providers of aged services for the region.<\/p>
This move allows the CHSP services to continue to have home support workers, and most importantly those who rely on the services provided.\u00a0<\/p>
The CHSP provides small amounts of entry-level support to assist older people aged 65 years and\u00a0over (50 years and over for Aboriginal and Torres Strait Islander people) to remain living at home\u00a0and in their community.<\/p>
The CHSP funds domestic assistance, transport, meals, personal care, home maintenance, social\u00a0support, nursing, and allied health. The CHSP also supports care relationships through planned\u00a0respite services for older people. These respite services allow carers to take a break from their usual\u00a0caring responsibilities.<\/p>
The program places a strong focus on activities that support independence and social connectedness\u00a0and consider each person\u2019s individual goals and choices.<\/p>
