OUR SERVICES

Community Aged Care Services (CACS)

Portland District Health provides Commonwealth Home Support Programme (CHSP) services for people aged 65 years and over, and Aboriginal and Torres Strait Islander people aged 50 years and over. These services are designed to support individuals to remain living independently, safely, and actively within their own homes and communities.

The program focuses on promoting independence, wellbeing, and social connectedness by supporting activities that align with each person's goals, preferences, and choices. Staff work collaboratively with clients, families, and carers to understand individual needs and deliver person-centred support.

Services available include domestic assistance, meals on wheels, personal care including shower assistance, and flexible respite. Social support and community engagement services can assist with attending appointments, shopping, bill paying, and participating in individual or group social activities.

Fees are discussed during the admission process and are based on the services provided.

Contact: phone (03) 5521 0353, email chsp@swarh.vic.gov.au, Monday to Friday, 8:00am to 5:00pm (excluding public holidays).
