The Chronic Disease Management Programs (CDMP), which include Pulmonary Rehabilitation and Cardiac Rehabilitation, are managed by the Chronic and Complex Care team. The service aims to assist people with chronic diseases to become independent at home with a focus on self-management. Programs are delivered either in group settings or through individual appointments based on patient needs.
