The Diabetes Education Service supports people living with diabetes to develop the knowledge, skills, and confidence needed to effectively self-manage their condition. Diabetes Educators work collaboratively with general practitioners, endocrinologists, and other members of the multidisciplinary healthcare team to develop individualised care plans and achieve shared health goals.
The service provides education, support, and practical guidance across all aspects of diabetes management.
Our aim is to empower people living with diabetes to make informed decisions, build self-management skills, and achieve the best possible health outcomes.
