OUR SERVICES
HARP is a coordinated multidisciplinary team approach to managing people with complex care needs and chronic health conditions that affect how you or your family cope at home, such as heart disease, respiratory disease or breathing complaints, arthritis, diabetes, memory issues, or psychological needs.
HARP aims to decrease avoidable hospital admissions and emergency department use by supporting you to identify the health and wellbeing areas that could be improved, teaching you to understand warning signs that you're getting ill, and linking you into community services you may need. It's targeted at people who either frequently present, or are at risk of presenting, to hospital because of chronic disease or complex medical and/or social issues. The program provides free short-term coordination, case management and education by developing an action plan with you and your support team, and linking people to longer-term community supports where appropriate.
Who can use this service? What is the cost? Eligible clients are people who are at imminent risk of hospitalisation, or who have had an avoidable hospital admission in the past twelve months and require moderate-to-intensive care coordination. There is no cost for this service.
Who do I see? The HARP Team are Registered Nurses who can do home visits, support over the phone, and have appointments at the PDH Consulting Clinic. Your Care Coordinator works closely with your GP, specialists and other care providers to give you the best of care.
The Hospital Admission Risk Program (HARP) operates Monday to Friday, 8:30am to 5:00pm. All referrals are managed through the Intake team, who acknowledge receipt of every referral. To find out more or make a referral, contact the Intake team on (03) 5522 1435 or email intake.pdh@swarh.vic.gov.au.
