OUR SERVICES
District Nursing
The District Nursing Service provides comprehensive in-home nursing care to support clients in maintaining their health, wellbeing, and independence. Services are available to people of all ages with a medical condition, illness, or disability who choose to receive care in their own home.
Our experienced nursing team provides a wide range of clinical and supportive care services, including:
- Wound assessment and management
- Catheter care and support
- Stomal care
- Palliative care support
- Diabetes management
- Medication management and administration support
- Care and management of Central Venous Access Devices, including:
- PICCs (Peripherally Inserted Central Catheters)
- Ports
- Hickman catheters
- Coordination of care with other healthcare providers and services
Fees
Any applicable fees will be discussed during the initial assessment and admission process.
The District Nursing Service provides care within Portland and a 10 km radius. Care for people living outside these designated boundaries may be negotiated on an individual basis.
The service operates 7 days a week, 8am to 8:30pm.
Referrals can be submitted via TrakCare and should be preceded by a telephone call to the District Nursing Service. Referrals are accepted from all sources.
Eligibility for services is determined following a District Nursing Service assessment.
A written referral from a medical practitioner is required when medical treatment is needed.
