The Victorian Freedom of Information Act 1982 grants individuals the right to request access to documents held by Portland District Health (PDH) and to request an amendment to information about them that they believe is incorrect or misleading. PDH maintains various document types including policies, procedures, reports, medical records, correspondence, meeting records, financial records, staff records, and archival materials. Certain documents may be exempted, such as those containing personal information about other people, information provided in confidence, or internal working papers.
Requests must be submitted in writing using the official form and sent to the FOI Officer via email (foi.pdh@swarh.vic.gov.au), phone (5521 0341), or delivered in person to the Main Reception Desk. Applications require a clear description of the requested documents, the application fee ($34.50), photo identification, and proof of relationship or consent for third-party medical records.
PDH must respond within 30 calendar days of receiving a valid application. Additional access charges apply for photocopying (20 cents per page), retrieval fees ($15.00), and postal delivery ($15.00).
Applicants who are dissatisfied with a decision may seek review through the FOI Commissioner, the Health Services Commissioner, or the Victorian Civil and Administrative Tribunal.
