The aims of Health Independence Programs (HIP) is to create an integrated, effective and sustainable chronic disease and complex needs programs of care, to provide the right care in the right setting, to reduce avoidable hospital admissions and contribute to better health outcomes for the Glenelg Shire Community.
The HIP guidelines have been developed by the Department of Human Services to provide direction and facilitate alignment for health independence programs that include:
